How to Fill Central Bank of India ATM Card Application?

So, follow below steps:

1. Write your branch name. (Where you have your account)
2. Mention the account holder name in capital letter.
3. So, write your father/husband name.
4. Next, write Date of Birth (DOB) in number 4.
5. Tick mark on the gender option (Either male or female)
6. If you have ATM card in Central Bank of India account, mention the details of it. (It is optional)
7. Permanent address of the account holder.
8. Next, present address of the account holder.
9. Fill your mobile registered mobile number.
10. In 10, enter your email address.
